Fostering Recovery program

Together, Libertas, American Foundation of Counseling and Advocates for Healthy Transitional Living offer a unique model of care for adolescents with substance use disorder who are in need of foster care after being in the inpatient setting. It’s a system of specialized foster care homes with additional training in substance use recovery strategies – Fostering Recovery. 
 
With Fostering Recovery, adolescents will spend 2 to 3 weeks at Libertas for detox, stabilization, assessment and treatment and discharge planning. While working with the patient’s social worker, Libertas and American Foundation of Counseling or Advocates for Healthy Transitional Living will develop and facilitate a discharge plan for this specialized foster care program. 
 
After approval has been given to participate in Fostering Recovery, Libertas will help the patient navigate through inpatient treatment.
  • Libertas treatment includes medical, substance use and mental health assessments.
  • Foster care social workers and foster families may attend family programming and preparation with the adolescent at Libertas to facilitate a seamless transition from inpatient services to foster care.
  • Follow up care planning for one of the following will be put in place so the adolescent will be able to continue substance use treatment at Libertas while living with his or her foster family.
    • Intensive Outpatient
    • Outpatient
    • After Care
  • Regular communication and updates will be provided to the social worker who placed the adolescent in the foster care program.
